  • Listen actively so care is unbiased and responsive to each person's needs.

Communities served

Near North Health centers are located in federally designated Health Professional Shortage Areas and Health Services Shortage Areas for primary care, mental health, and dental services.

The communities served include African American, Latinx, and other racial and ethnic groups who experience higher rates of conditions such as:

  • Diabetes
  • Heart disease
  • Asthma
  • HIV
  • Depression
  • Tobacco use
  • Infant mortality and low birth weight

Near North Health's goal is to reduce these health disparities and improve health, well-being, safety, and quality of life across Chicago's neighborhoods.
