Patient-Centered Medical Home (PCMH)

Santo Domingo Health Center follows a Patient-Centered Medical Home (PCMH) model.

What PCMH means for patients

A Patient-Centered Medical Home is a partnership between you and your primary care team. It focuses on providing:

  • Personalized care plans that you help design
  • Medication review so you understand your prescriptions
  • Coaching and advice to help you follow your care plan and meet your health goals
  • Coordinated care within the clinic and with outside referrals
  • Support and encouragement from care teams and community resources
Primary care providers and care teams
  • Each patient is assigned a primary care provider to improve continuity of care.
  • This helps ensure better communication, stronger relationships, and consistent follow-up for prevention and chronic disease management.
  • If your assigned provider is unavailable, you may be seen by another provider on the same team or by another available provider.
Care teams
  • Patients are grouped into Red Team or Blue Team based on their primary care provider.
  • All pediatrics patients are assigned to the Pink Team.

Your care team may include:

  • Primary care providers (physicians or advanced practice providers)
  • Registered nurses
  • Medical assistants
  • Other clinical and support staff

The goal is to provide safe, comprehensive, and coordinated care where you and your family are the focus.

When to see the dentist

Routine dental check-ups

At regular check-ups, your dental professional may:

  • Check for cavities, plaque, and tartar
  • Examine your gums using a special tool to measure the spaces between teeth and gums
  • Examine your tongue, throat, face, head, and neck for swelling or other concerns
  • Discuss any concerns and schedule or refer you for needed treatment
Professional dental cleanings

Brushing and flossing at home removes plaque but not hardened tartar. During a cleaning:

  • The dental team removes tartar using specialized instruments (scaling)
  • Teeth are often polished with a gritty paste to remove surface stains
  • Flossing ensures the areas between teeth are thoroughly cleaned
If you have dental pain or symptoms

You should contact the dental clinic if you have:

  • Tooth pain or sensitivity
  • Headache, earache, or jaw pain lasting more than a few days
  • Visible damage, swelling, or trauma in the mouth
