History
- Founded in 1977 as Claretian Medical Center at 91st and Commercial Avenue
- Created in partnership with local residents, community stakeholders and the Claretian priests of Our Lady of Guadalupe Church to provide a health safety net after the decline of the steel industry
- Renamed Chicago Family Health Center in 1998 to reflect its broader community mission
- Now operates multiple health centers and serves more than 30,000 patients each year.
